Inclusion Criteria: Accepts referrals for any patients presented and located in the acute floor with the following criteria:

  • 65 years of age and over.

  • Variable Indicative Placement (VIP) Positive at triage.

  • Clinical Frailty Score (Rockwood frailty) between 4-9.

  • Presenting to the ED with one or more of the following frailty syndromes and presenting complaints; Falls, Syncope, Poor mobility, Delirium, Dementia, Cellulitis, Leg Ulcers, Wound care, General deconditioning/Sarcopenia, Stable fractures, Bone health, Vertigo, Polypharmacy adverse effects, Complex social circumstances.

Exclusion Criteria:

  • Patients less than 65 Years old.

  • Patients with higher acuity or critical care needs.

  • Patients who are for scheduled return by a different specialty (i.e. TIA clinic).
