Frailty at the Front Door
Inclusion Criteria: Accepts referrals for any patients presented and located in the acute floor with the following criteria:
65 years of age and over.
Variable Indicative Placement (VIP) Positive at triage.
Clinical Frailty Score (Rockwood frailty) between 4-9.
Presenting to the ED with one or more of the following frailty syndromes and presenting complaints; Falls, Syncope, Poor mobility, Delirium, Dementia, Cellulitis, Leg Ulcers, Wound care, General deconditioning/Sarcopenia, Stable fractures, Bone health, Vertigo, Polypharmacy adverse effects, Complex social circumstances.
Patients less than 65 Years old.
Patients with higher acuity or critical care needs.
Patients who are for scheduled return by a different specialty (i.e. TIA clinic).
