This proforma follows national and international guidelines in relation to trauma care and is underpinned by the standards set out in the Major Trauma Audit including use of tranexamic acid; most senior doctor review patients with an Injury Severity Score(ISS) >15; Patients with GCS <9 to have definitive airway management
